Change in Immunization Recordkeeping
As of April 2023, Kent School District is using School Module, an online system provided by Washington State Department of Health, to manage student immunization records. Most children born and/or vaccinated in Washington State already have their information in the system. You can access your child’s immunization record at any time by signing up for MyIR.
As we move to this system, you may be asked for more information about your child’s vaccination history. If your child is missing vaccines in the system, we may ask for a copy of their immunization record and may be able to assist in getting those vaccination records entered into the online system.

Vaccine Requirements for New Students
All new students enrolling in Kent School District are required to provide medically verified immunization records.
This means immunization records turned into the school for incoming students must be from a health care provider, or paperwork from a health care provider must be attached to a handwritten form showing your child’s records are accurate.
A Certificate of Immunization Status (CIS) printed from Washington MyIR. Once you create an account on MyIR, you will be able to access the certificate at any time.
A physical copy of the CIS form with a healthcare provider signature.
A physical copy of the CIS form with accompanying medical immunization records from a healthcare provider verified and signed by school staff.
A CIS printed from the Washington State Immunization Information System by the school nurse or other school staff.

Vaccine Requirements
Washington State law requires all students to be fully immunized against chicken pox, hepatitis B, diphtheria, tetanus, polio, measles, pertussis, mumps, and rubella before attending school.
Five doses of tetanus-diphtheria (fewer doses may be accepted depending on child’s age and dosage spacing).
Four doses of polio (three doses may be accepted if the third dose was given after child’s fourth birthday and correct dosage spacing).
Two doses of the mumps, measles, and rubella (MMR) vaccine after age one and at least one month apart.
Hepatitis B series with the final dose being given after child was six months old and meeting minimal dosage spacing.
Two doses of varicella (chickenpox) after age one and at least one month apart or written verification of the disease from a licensed health care provider.
Tdap before entering seventh grade and when child is eleven years of age.
Exempt Status
To request your child to be exempt from any immunization requirement, you must have a completed Certificate of Exemption (COE) form on file, which may be obtained from your child’s school or health care provider.
Students with exempt status will be excluded from school in the event of an outbreak and will remain excluded per Department of Health recommendations. One case constitutes an outbreak.

Parents and guardians must provide a record of student’s immunizations by completing a Certificate of Immunization Status (CIS) form available from the Washington State Department of Health in multiple languages.
